Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were likely to cause death or serious physical harm to an employee in
that tripping out
of the hole was permitted while methane was flowing out of the well and
onto the rig floor:
a.Well #2: Drill string and bottom hole assembly were being tripped out of
the
hole while methane was flowing out of the well and into the substructure
and
rig floor areas, exposing employees to a fire hazard as evidenced by an
ignition, fire, and burns that occurred June 7, 2010.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is when tripping out of the hole and pressurized methane gas is
encountered,
reinstall the air bowl rubber, when possible. If it is not possible to
reinstall the air
bowl rubber, employees are to be trained and instructed to shut the rig
down, safely
remove all ignition sources, and evacuate the area immediately.
