were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck-by and crushing hazards:
(a) Location - Hi Bay Area:
On or about 6/23/09, the employer failed to ensure frequent inspections of
two (2) 10-ton
bridge cranes.
Among other methods, one feasible and acceptable method to correct this
hazard is to train employees to inspect the crane and hoist mechanism in
accordance with the most
applicable version of ASME B30-18.
Daily or before each use (daily to monthly intervals:
All functional operating mechanisms for maladjustment interfering with
proper operation.
Deterioration or leakage in lines, tanks, valves, drain pumps, and other
parts of air or
hydraulic systems. Rope reeving for noncompliance with manufacturer's
recommendations.  Hoist chains, including end connections, for excessive
wear, twist,
distorted links interfering with proper function, or stretch beyond
manufacturer's
recommendations. Visually inspect daily; monthly inspection with a
certification record
which includes the date of inspection, the signature of the person who
performed the
inspection and an identifier of the chain which was inspected.
Hooks with deformation or cracks. Visually inspect daily; monthly
inspection with a
certification record which includes the date of inspection, the signature
of the person who
performed the inspection and the serial number, or other identifier, of
the hook
inspected.
Discard hooks with cracks or having more than 15 percent in excess of
normal throat
opening or more than 10 percent twist from the plane of the unbent hook
