Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees:
a) On or about 10-30-09 on the rooftop of building 215, employees were
connected to
a vertical lifeline that was tied in knots at the anchorage point exposing
the employees
to a 100 feet fall hazard.
Among other methods, one feasible and acceptable means Is to comply with
ANSI
A10.32-2004, Section 3.7.3, which states, Knots are not allowed in
lifelines, lanyards
or other direct impact components.
