1970: The employer
did not furnish employment and a place of employment which were free from

that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of fire/deflagration from combustible
dust more than 1/32
of an inch in depth:
A) T-Shed 1, on or about March 25, 2011, employees who work in the area
where
combustible dust more than 1/32 of an inch in depth was observed on over
head pipes
and on horizontal surfaces along the wall on columns and on the floor and
use
compressed air and blowers to remove the dust are exposed to a
fire/deflagration hazard.
B) T-Shed 2 and 3, on or about March 25, 2011, employees who work in the
area where
combustible dust more than 1/32 of an inch in depth was observed on over

develop a cleaning schedule that will keep the dust accumulation less than
1/32 of an inch
in depth on horizontal surfaces.
