Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: Being struck-by and/or crushed-by moving vehicle traffic
in temporary traffic
control zone.
On or about 06/30/2011 at Hwy 29 and Harmony Grove Road, Lilburn, GA:
Employees
installing new utilities were exposed to being struck by movng vehicles.
Employees did not
follow standard MUTCD safety procedures; a transition area ( with correct
lenght and cone
placement) was not created to safely move traffic from a normal two lane
path to one lane only.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included
feet apart.
b) the length of the traffic control zone was approximately 100 feet,
instead of 500 feet.
c) signs were place at the end of the traffic control zone approximately
80 feet from where the
men were working instead of at the beginning of a properly constructed
traffic work zone that
should have been calculated on the speed limit.
d) the merging taper was 100 feet long instead of 1000 feet to enable
merging drivers to have
enough time to adjust their speed and merge into an adjacent lane before
the downstream end.
Among other methods, one feasible and acceptable method to correct this
hazard is to follow the
Manual of Uniform Traffic Controls Devices (MUTCD).
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
