Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of being struck by:
a)  On or about 02/14/11, and at times prior there to, employees were
exposed
to being struck by collapsing precasted concrete members.  The employees
were
in the process of erecting the skeleton frame of a North Prow, a 90' x 172'
section of the parking garage.  The employee had erected a portion of this
section
approximately 7 levels high.  During the erection process the employer
failed to
follow the erection notes and ensure that the columns were dry packed
immediately after erection.
b)  On or about 02/14/11, and at times prior there to, employees were
exposed
to being struck by collapsing precasted concrete members.  The employees
were
in the process of erecting the skeleton frame of a North Prow, a 90' x 172'
section of the parking garage.  The employee had erected a portion of this
section
approximately 7 levels high.  During the erection process the employer
failed to
follow the erection notes and ensure that no more than 2 levels were
erected
before columns were fully grouted.
c)  On or about 02/14/11, and at times prior there to, employees were
exposed
to being struck by collapsing precasted concrete members.  The employees
were
in the process of erecting the skeleton frame of a North Prow, a 90' x 172'
section of the parking garage.  The employee had erected a portion of this
section
approximately 7 levels high.  During the erection process the employer
failed to
follow its bracing plan and ensure that the columns were brace in two
directions
at 90 degrees until grouted and tied in with other members.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement is to follow
the erection notes, in
the plans, which specific when to grout and dry pack the column splices.
Also the employer
developed an site specific bracing plan.  The company shall ensure that
they brace the structure
in accordance with the accepted bracing plan.
