support personnel:
(a) On or about July 17, 2010 - Resin Plant, the employer did not ensure
that
contractor employees engaged in changing stainless steel pipes and flanges
followed safe work practices addressing hazards such as, but not limited
to,
lockout/tagout of equipment; hot work permits; the opening of process
equipment
or piping; confined space entry; and control over entrance into a facility
by
maintenance, contractor, laboratory, or other support personnel.
