Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees employment and a place of employment
which were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to fire and/or explosion
hazards:  This
condition occurred on or about December 2, 2010, at the ELLIE LEASE-WELL
#1415.
The employer did not ensure that appropriate equipment was provided,
maintained, serviced,
and/or inspected.  Among other methods, feasible and acceptable means of
abatement for this
hazard include, but not limited to, implementing the measures outlined in
the American
Petroleum Institute (API) Recommended Practice (RP)54.  Specifically,
Chapter 6,
Operations and Paragraphs 6.1.1 and 6.4.4.
