employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
Employee was not wearing a seatbelt when operating the Sky Trak forklift.
A feasible means of abating this hazard is for the employer to enforce use
of seat belts by
equipment operators.
