Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fractures, lacerations, and internal injuries from
rotating and/or moving drills
and drill components:
In the following instances energy control procedures were not instituted
to protect employees
from the hazards of rotating and moving drills and drill components:
a)Rollins Wind Farm, Pad 19 - On or about 10/28/2010, an employee was
seriously
injured due to entanglement with a rotating drill.  The employee was
wearing
loose clothing and administrative controls had not been instituted.
b)Rollins Wind Farm, Pad 19 - On or about 10/28/2010, an employee was
seriously
injured due to entanglement with a rotating drill.  During a drill bit
change,
the
drill bit was positioned in close proximity to the drill operating
controls and
energy control procedures had not been instituted.
c) Rollins Wind Farm, Pad 19 - On or about 10/28/2010, an employee was
seriously
injured due to entanglement with a rotating drill.  The drill rotation
control was
initiated while the employee was in close proximity to the drill and
return to
service procedures had not been instituted.
d)Rollins Wind Farm, Pad 19 - On or about 10/28/2010, during a drill bit
change
the bit was threaded by hand while the employee activated the rotation
control
and equipment isolation had not been achieved.
e)Rollins Wind Farm, Pad 19 and Access Road - On or about 10/28/2010 and
11/3/2010, during drill bit changes the lockout and/or tagout devices were
not
affixed to the drill's energy isolation device in a manner that would
ensure that
the drill remained in a safe or off position.
f)Rollins Wind Farm, Pad 19 and Access Road - On or about 10/28/2010 and
11/3/2010, during drill bit changes the elevated drill attachment
(containing the
rod, feed, hose drum and drill apparatus) was not grounded, blocked, and/or
isolated.
Among other methods, one feasible and acceptable means of abatement would
be to institute
procedures for the control of hazardous energy as prescribed in ANSI/ASSE
Z244.1-2003
