Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in hazards while emptying a mixer with unidentifiable emergency stop controls. On or about April 22, 2011, the Kneader tilt mixer emergency stop actuators located at the discharge end were not colored red, did not have a yellow background, and were not of the palm or mushroom-head type. Employees were exposed to a caught-in hazard created by moving rotors when transferring rubber from the discharge end of the Kneader tilt mixer to the bucket conveyor. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the guidelines in National Fire Protection Association (NFPA) 79, discharge end were not colored red, did not have a yellow background, and were not of the palm or mushroom-head type. Employees were exposed to a caught-in hazard created by moving rotors when transferring rubber from the discharge end of the Kneader tilt mixer to the bucket conveyor. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the guidelines in National Fire Protection Association (NFPA) 79, 2007 Edition, Electrical Standard for Industrial Machinery, Chapter 10.7 Devices for Stop and Emergency Stop as applicable.
