North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall from elevation: a)Fall protection system lanyard located on the facilities "Skyjack" SJIII 3219 scissor lift - The lanyard for the fall protection system, which the employer required to be used while operating scissor lift, had burns in the deceleration device and synthetic webbing of the lanyard. b)Fall protection system lanyard located on the facilities "Skyjack" SJIII 3219 scissor lift - The lanyard for the fall protection system, which the employer required to be used while operating the scissor lift, was looped around the lifts guardrail and attached back to itself. Abatement Note: Remove the lanyard from service and replace with a new lanyard. Enforce that employees inspect the fall protection system prior to each use and if any defects are noted remove the system from service and replace with a new fall protection system. Company policy should reflect that fall protection systems are inspected prior to each use.
