Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to a fall and crush hazard due to being elevated on
a wooden pallet
by a rough terrain forklift:
a)for employees performing pre-construction work at the site located at
1127
Bassage Road in Sebring, (FL), on but not limited 12/02/2011.  In that two
employees were removing moss and trimming trees while standing on a make
shift wooden platform elevated by a Mustang 642 Telehandler (SN
642JW1280149).  The rough terrain forklift was operated on rough, sloped,
sandy terrain in windy conditions.
Abatement Note(s): A feasible and acceptable means of abatement would be to
follow the guidelines set forth in ANSI/ITSDF B56.6-2011, Safety Standard
for Rough terrain Forklift Trucks, and the Operator's Manual.  The employer
should comply with Sections 5.15 Elevating personnel, such as:
(a) provide a platform which complies with the design
requirements listed in Part III (page 35 of manual);
(b) be ceratin that the platform is securely attached to the lifting
carriage (page 36 of manual);
(h) be certain that mast or boom travel is vertical - do not
operate on a side slope unless the rough terrain vehicle is
leveled (pages 26 and 32 of manual); and,
(j) be certain the rough terrain has a firm footing.
