furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious
physical harm to employees:

Location:South Side of Building, Courtyard
Equipment:Sperian Fendall Model #32-000-200-0000 Self-Contained Eye Wash
Condition:Where employees were using LaFarge Ready Mix Concrete with a pH
of 12 to 13, a
Sperian Fendall 16 gallon self-contained eyewash station provided for
employee use,
was not inspected, tested, and maintained in accordance with the
manufacturer's
recommendations.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to inspect, test,
and maintain an eyewash in accordance with American National Standards
Institute ANSI Z358.1-2009
requirements, or the eyewash manufacturer's recommendations.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
