The 120 volt outlet was zip tied to the rack and hanging off of the rack.  The outlet was used to power lights and the reverse osmosis unit in the south flower area.  This condition exposed the employees to electrical shock hazards.\r\n\r\n(b) Beyond Broadway LLC at 5141 Franklin St. in Denver, CO: On and before 6/10/11, the employer did not ensure the electrical outlet attached to the south rack in flower area C was firmly secured to the surface on which it was mounted.  The 120 volt outlet was zip tied to the rack and tilted on an angle.  The outlet was used to power lights and fans.  This condition exposed the employees to electrical shock hazards.\r\n\r\n(c) Beyond Broadway LLC at 5141 Franklin St. in Denver, CO: On and before 6/10/11, the employer did not ensure the electrical outlet used to power lights and fans for flower area D was firmly secured to the surface on which it was mounted.  The 120 volt outlet was lying on the ground between flower areas C and D.  This condition exposed the employees to electrical shock hazards.\r\n\r\n(d) Beyond Broadway LLC at 5141 Franklin St. in Denver, CO: On and before 6/10/11, the employer did not ensure the Harvest Pro remote ballasts were firmly secured to the surface on which they were mounted.  The ballasts were hanging by metal chains over the vegetative area.  The ballasts operate at 240 volts 60 hertz and are used to power 1000 watt metal halide lamps in the first vegetative area next to the nursery.  This condition exposed the employees to electrical shock hazards.\r\n\r\n(e) Beyond Broadway LLC at 5141 Franklin St. in Denver, CO: On and before 6/10/11, the employer did not ensure the Harvest Pro remote ballasts were firmly secured to the surface on which they were mounted.  The ballasts were placed on a wooden platform above the flower areas.  The ballasts operate at 240 volts 60 hertz and are used to power 1000 watt high pressure sodium lamps in the flower areas.  This condition exposed the employees to electrical shock hazards.\r\n\r\n(f) Beyond Broadway LLC at 5141 Franklin St. in Denver, CO: On and before 6/10/11, the employer did not ensure the temporary power tap near the reverse osmosis unit was firmly secured to the surface on which it was mounted.  The 120 volt office type surge protector was hanging by the multiple flexible cords that were plugged into the unit.  The temporary power tap was used to power lights and the reverse osmosis pumps.  This condition exposed the employees to electrical shock hazards.\r\n\r\n(g) Beyond Broadway LLC at 5141 Franklin St. in Denver, CO: On and before 6/10/11, the employer did not ensure the temporary power tap hanging on the wall in the southwest vegetative area was firmly secured to the surface on which it was mounted.  The 120 volt office type surge protector was hanging on the wall by the flexible power cord.  The temporary power tap was used to power fans.  This condition exposed the employees to electrical shock hazards.\r\n\r\n(h) Beyond Broadway LLC at 5141 Franklin St. in Denver, CO: On and before 6/10/11, the employer did not ensure the temporary power tap hanging from the ceiling in the nursery area was firmly secured to the surface on which it was mounted.  The 120 volt office type surge protector was hanging from the ceiling by the flexible power cord.  The temporary power tap was used to power the reverse osmosis unit and fans in the nursery area. The employer requires half-face 3M 6300 respirators while spraying M-Pede and other pesticides.
