Any deficiencies such as listed were not carefully examined and determination made as to whether they constitute a safety hazard:    a) Doral Steel, Inc. located on 1500 Coining Drive in Toledo, OH:  On or about June 13, 2011 the employer hired a third party agency to perform a periodic crane inspection and found deficiencies that constituted a safety hazard but has no documentation that deficiencies, such as but not limited to, frayed hoist cables and chains were repaired on a 10 ton Shawbox crane (model 84-1141).       b)  Doral Steel, Inc. located on 1500 Coining Drive in Toledo, OH:  On or about June 13, 2011, the employer hired a third party agency to perform a periodic crane inspection and found deficiencies that constituted a safety hazard but has no documentation that deficiencies, such as but not limited to, no upper limit switch and cracked welds were repaired on a 10 ton Michigan crane (model 73-1121-A).    c) Doral Steel, Inc. located on 1500 Coining Drive in Toledo, OH:  On or about June 13, 2011, the employer hired a third party agency to perform a periodic crane inspection and found deficiencies that constituted a safety hazard but has no documentation that deficiencies, such as but not limited to, broken wires, no bumpers, and loose j-bolts were repaired on a 7.5 ton Michigan crane (model 66-1949).    d) Doral Steel, Inc. located on 1500 Coining Drive in Toledo, OH:  On or about June 13, 2011, the employer hired a third party agency to perform a periodic crane inspection and found deficiencies that constituted a safety hazard but has no documentation that deficiencies, such as but not limited to, loose brake shoes and frayed hoist cable were repaired on a 20 ton Michigan crane (model 73-l
