On or about August 5, 2011, at the above addressed jobsite, an employee was cutting sewer pipe using a hand held grinder.  The guard was not installed on the grinder, thereby exposing the employee to lacerations.

On or about August 5, 2011, at the above addressed jobsite, an employee installing a sewer line in a trench, nine (9) feet in depth, was not provided a safe means of ergress.

On or about August 5, 2011, at the above addressed jobsite, employees were installing a sewer line in a trench measured at nine (9) feet in depth.  The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.
