OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to fall hazards: a) On or about August 9, 2011 and times prior, employees were exposed to serious injuries, in that, the wire rope attachments used as an anchorage for the lifeline were not engineered or evaluated to determine their capacity. This condition was found on the north leg head on the east roof and on the flat storage roof. Abatement Verification: Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").
