29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.      a) Precision Thermoplastic Components, Inc. located at 3765 St. Johns Road in Lima, OH: On or about September 1, 2011, the employer did not remove the Toyota forklift model 42-6GFCU15 Serial #63276 from use based on mechanical defects such as but not limited to, broken headlights that exposed employees to unsafe operating conditions.

Specifically, column F did not list information explaining the objects or substances that directly resulted in the injury or illness.
