29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: (a) Inside the structure near the east wall on or about September 12, 2011 and at times prior thereto, there was no grounding prong on an extension cord pig tail with three female plugs/receptacles. The pig tail was connected to a spider box with ground fault circuit-interrupters that functioned and supplied power to 110 VAC battery charges.
