29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a) Chita Roofing @ 62927 US Hwy 40, Granby, CO 80446: On 9/20/11, the employer did not ensure that safety inspections regarding the use of personal fall arrest systems were conducted. Employees conducted roofing activities on a 5 in 12 pitch roof without fall protection. This condition exposed four subcontracted employees to a fall hazard in excess of 25ft.

(a) Chita Roofing @ 62927 US Hwy 40, Granby, CO 80446: On 9/20/11, the employer did not ensure that employees worked on a 5 in 12 pitch multi-family residential roof with personal fall arrest systems. This condition exposed four subcontracted employees to a fall hazard in excess of 25ft.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) Chita Roofing @ 62927 US Hwy 40, Granby, CO 80446: On 9/20/11, the employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees engaged in roofing activities on a 5 in 12 pitch roof without the use of a personal fall arrest system. This condition exposed employees to a fall hazard in excess of 25ft.
