(A) A service vehicle located adjacent to the Auto Stack area, contained maintenance equipment and other materials which obstructed means of access and egress to the rear entrance of the service vehicle, thereby exposing employees to slip, trip, and fall hazards that may potentially result in contusions and/or lacerations.

29 CFR 1917.11(d): Dunnage, lumber, or shoring material had protruding nails that were not removed from the immediate work area. (A) At the Horizon Lines marine terminal, adjacent to Matson fence and electrical area, lumber with protruding nails were not removed or rendered harmless from the immediate work area, therby creating a laceration and/or impalement hazard.

29 CFR 1917.151(g)(1): The safety guard or the tongue was not adjusted so that they were always close to the periphery of the wheel. (A) At the maintenance shop, the tongue guard of the 3/4" bench grinder was not adjusted so that the wheel is always close to the periphery of the wheel, thereby exposing employees to flying fragments from wheel, potentially resulting in imbedded fragment in skin.
