(a) At the 9704-9706 W. Lincoln Avenue, West Allis, Wisconsin jobsite, an employee was performing residential roofing activities on an approximate 4/12 pitch roof without adequate fall protection. The employee was exposed to a 19.6 foot fall hazard.

Lincoln Avenue, West Allis, Wisconsin jobsite, the Werner extension ladder used to gain access to the upper roof landing was extended approximately 1 rung above the landing. Employees were exposed to approximately 19.6 foot fall hazards.
