29 CFR 1910.106(e)(6)(i):  Adequate precautions were not taken to prevent the ignition of flammable vapors:    a)  On or about November 16, 2011 and at times prior, employees were exposed to explosion and fire hazards in that, adequate precautions were not taken to ensure that containers of flammable liquids were stored or so located so that vapors from these containers could not be released and ignited.  Flammable liquids including a 5 gallon can of gasoline and flammable paints were stored in a box truck compartment next to or near heat sources from a welder and compressor.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    a)  On or about November 21, 2011 and at times prior, spare oxygen cylinders were stored next to fuel gas cylinders in the welding trucks.
