29 CFR 1910.23(a)(8)(ii): A floor hole into which persons can accidentally walk was not guarded by a floor hole cover of standard strength and construction: a) On or about October 25, 2011, and times prior, Marathon Oil Corporation, as controlling/exposing employer, did not ensure that their employee and Helmerich & Payne International Drilling Co., dba H&P, Inc., RWDY, Inc., subcontractors, protected their employees from injury in that, the cellar cover had 2 foot by 2 foot opening.

29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level was not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v): a) On or about October 25, 2011, and at times prior, Marathon Oil Corporation, as controlling/exposing employer, did not ensure that their employee and Helmerich & Payne International Drilling Co. dba H&P, Inc., RWDY, Inc., subcontractors, protected their employees from falls approximately 26 feet while working on the rig drill floor in that, the V-door was left open.
