29 CFR 1926.502(d)(15):  Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds per employee attached, nor was it designed, installed and used as follows:  as part of a complete personal fall arrest system which maintains a safety factor of at least two, and under the supervision of a qualified person:    (a)  Big Johnson Construction, LLC at 3410 Second Avenue West, Williston, ND 58701:  On or about October 28, 2011, the employer did not ensure that the two roof anchorages used with the two retractable lanyards in the installation of the roof decking were capable of supporting loads from falls, in that each roof anchor was connected to the roof deck with only two screws.  Condition exposed employees to fall hazards of approximately 23 feet.    Abatement note:  One feasible means of abatement is to use the roof anchors in accordance with the manufacturer's recommendations.

29 CFR 1926.760(a)(1):  Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:    (a)  Big Johnson Construction, LLC at 3410 Second Avenue West, Williston, ND 58701:  On or about October 28, 2011, the employer did not ensure that employees were protected from falling when rolling out the roofing insulation and when setting the metal roof deck.  The employees were not tied off.  In addition, the horizontal lifelines (wire rope) used for laying the insulation were spaced too far apart for the employees to connect their lanyards and still provide fall protection for all operations on the roof.  Condition exposed employees to fall hazards of approximately 23 feet.    Big Johnson Construction, LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent, which was contained in OSHA Inspection Number 314023441, Citation Number 1, Item Number 3, issued May 11, 2010, and became a final order on June 2, 2010, with respect to a workplace located at Pine and East Highway 54, Kingman, KS 67068.
