29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules,and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, thefollowing: a.) As of and prior to, December 9, 2011, the employer had not established specific procedures for hazardous energy control in Plant 5 for the Shear Line. There were no procedures to specifically address the specific steps for shutting down, securing and isolating the equipment; nor procedures to specifically address the placement and removal of devices; nor procedures for testing or verifying the effectiveness of lockout devices for the equipment associated with the Shear Line.
