29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section: a) On or about November 1, 2011 and at times prior, Sion Steel Erectors did not ensure that employees were protected from fall hazards. Employees constructing a prefabricated structure were working at heights above 15 feet without wearing any type of fall protection equipment to afford them the ability to tie off or were not provided with other means of fall protection.
