29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:    a) Jobsite: On January 4, 2012, and times prior thereto, employees were exposed to a fall of 19.5 feet while leaning over the West edge of a parapet wall that was only 36" with a 4" wall cap.    Note:  Abatement certification is required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    a) Jobsite: On January 4, 2012, and times prior thereto, employees were exposed to falls of 19.5 feet and were not trained to recognize that the vertical height of the parapet wall that measured 36 " needed to be 42" +/- 3".      Note: Abatement certification is required for this item.
