OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to:    (a) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to a struck by and crushing hazard while elevating vehicles with a hydraulic lift that was leaking oil from around the bolts on the end of the hydraulic cylinder on lift #1.      (b) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to a struck by and crushing hazard while elevating vehicles with hydraulic lift #1 due to having no load capacity rating listed on the lift.    (c) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to a struck by and crushing hazard while elevating vehicles with hydraulic lift #2 because the anti slip pads were missing on all four pads.     (d) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to a struck by and crushing hazard while elevating vehicles on hydraulic lift #2 because all four lift pads were bent.     (e) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to a struck by and crushing hazard while elevating vehicles on hydraulic lift #2 due to the upper cut off bar not operating.    (f) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to struck by and crushing hazard while elevating vehicles on hydraulic lift #2 due to having no load capacity rating listed on the lift.    (g) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to struck by and crushing hazard while elevating vehicles on hydraulic lift #2 because the cement around one of the legs was damaged.     (h) Auto repair shop: On or about January 20, 2011 and at times prior thereto, employees were exposed to a struck by and crushing hazard while elevating vehicles on hydraulic lifts due to not training employees in the proper use and care of the lifts.
