29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery,equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    a) Metal cover in the basement thats placed over the sump pump was not secured to prevent movment and opening in the cover allowed for trip and fall hazards.

29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall beguarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance toa ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a) Work platforms in the basement were not equipted with a guard railing system to prevent employees exposure to falls.

29 CFR 1910.219(c)(2)(i):     All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform,excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by atrough enclosing sides and top or sides and bottom of shafting as location requires.    a) The shaft on #1 leg was not guarded to prevent employees exposure to rotating parts.  b) The shaft on #2 leg was not guarded to prevent employees exposure to rotating parts.
