29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE) a) On or about 3/1/12 in the Area E Basement; during the August 2011 test of the repaired Automatic Transfer Switch ( ATS) #4 an arc flash occurred inside the closed cabinet. No injuries occured as a result of this incident. On Februrary 21, 2012 during testing of repairs to the ATS #4 an arc flash occured in side the cabinet. This event caused the closed cabinet doors to fly open and strike the employee conducting the test. The force knocked him to the ground resulting in a laceration to his forehead. Employee was not wearing eye or face protection. No Abatement Certification

29 CFR 1910.335(a)(1)(v): Employees shall wear protective equipment for the eyes or face wherever there is danger of injury to the eyes or face from electric arcs or flashes or from flying objects resulting from electrical explosion. a) On or about 3/1/12 in the Area E Basement; during a test of the repaired Automatic Transfer Switch #4 an arc flash occurred inside the cabinet.
