(a)  A stairway, ladder, ramp or other safe means of egress for the approximately 180 foot trench was not located in area's so as to require no more than 25 feet of lateral travel for employees working in the trench. One ladder was located on the far West end of the trench approx. 100' from where employees were working.    Mastec North America Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 29 CFR 1926.651 (c)(2), which was contained in OSHA Inspection #312138100, Citation 1, Item 2, which was affirmed as a final order on March 18th, 2009, with respect to the jobsite located at Nevada Ave. and Eagle Rock, Colorado Springs, CO 80909.    Mastec North America Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 29 CFR 1926.651 (c)(2), which was contained in OSHA Inspection #314303116, Citation 1, Item 1, which was affirmed as a final order on December 9th, 2010, with respect to the jobsite located at 12626 Cisco, San Antonio, TX 78247.
