29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork:    a)  At the worksite, at the dumpster, on or about 3/20/12:  An employee used an an all terrain forklift to elevate a wooden pallet with a wheelbarow full of rubble on it, above the top of a dumpster.  The employee then climbed the dumpster to step onto the pallet and pour the material in the wheelbarrow into the dumpster below, exposing the employee to a fall in excess of 6 feet.
