29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  Facility: The employer did not develop a lockout/tagout program which established the energy control procedures, employee training, and periodic inspections, for machines including but not limited to the molded cone machine and the rolled cone machine to ensure that before any employee performed any servicing or maintenance on those machines that the machine was isolated from the energy source and rendered inoperative, occurred on or about 4/17/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
