29 CFR 1910.133(a)(1):     The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Shale shaker area of the rig mud pits:  On or about April 9, 2012, an employee engaged in the operation and maintenance of the rig mud pits was not wearing protective eyewear.  This exposed the employee to the hazards of foreign objects and/or mud materials striking the eyes.

29 CFR 1910.169(b)(3)(ii):     Valve(s), of any type, were placed between the air receiver and its safety valve or valves:    Drilling rig substructure:  On or about April 9, 2012, employees operating a drilling rig during the drilling of a gas well worked around and with the rig air receivers.  One of the substructure mounted air receivers had a manual valve installed between the receiver and its relief safety valve.  This exposed the employees to the hazard of being struck by pieces of a rupturing air receiver.
