29 CFR 1926.304(d): Guarding. All portable, power-driven circular saws shall be equipped with guards above and below the base plate or shoe. The upper guard shall cover the saw to the depth of the teeth, except for the minimum arc required to permit the base to be tilted for bevel cuts. The lower guard shall cover the saw to the depth of the teeth, except for the minimum arc required to allow proper retraction and contact with the work. When the tool is withdrawn from the work, the lower guard shall automatically and instantly return to the covering position. (a) Right Side of House - On or about March 28, 2012 one employee was exposed to laceration and amputation hazards by using a Hitachi C7SB2 circular saw without automatic adjusting saw blade guard. The saw blade guard was blocked by a small block of wood. Therefore the saw blade guard could not automatically and instantly return to the cover position. (b) Front of House - On or about March 28, 2012 one employee was exposed to laceration and amputation hazards by using a Makita circular saw without automatic adjusting saw blade guard. The saw blade guard was blocked by a small block of wood. Therefore the saw blade guard could not automatically and instantly return to the cover position.

29 CFR 1926.405(j)(1)(i): Live parts. Fixtures, lampholders, lamps, rosettes, and receptacles shall have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 8 feet (2.44 m) above the floor may have exposed parts. (a) Right Side of House - On or about March 28, 2012 two employees were exposed to an electrical shock hazard with live electrical parts by using a portable Makta circular saw plugged into an open three receptacle outlet box. The outlet box had no cover and was lying directly on the ground where live electrical parts were exposed to moisture and insects.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. (a) Front of House - On or about March 28, 2012 two employees were exposed to a fall hazard up to 11 feet while climbing on a wooden single pole scaffold that did not have a ladder for access or egress. Employees used the scaffold frame to get on and off the scaffold.

Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.
