Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer does not furnish to each of his employees employment and a place of employment which are free from the recognized hazard of being electrically shocked by exposed circuits which are causing or likely to cause death or serious physical harm to employees:  This violation was observed on or about April 18, 2012, in the rear Paint Booth area where employees were exposed to an electrical hazard from using a Shaw-Box hoist pendant control station that was missing a power button exposing the energized electrical circuitry.  Among other feasible methods to correct the identified hazard, one method would be to replace the power button on the pendant control station to prevent inadvertent contact to live electrical parts per ASME B30.16-2007 Overhead hoists (Underhung) Para. 16-1.2.2(c).

This violation occurred on or about April 18, 2012, in the shop area where employees were exposed to electrical and struck-by hazards when they were allowed to perform service and/or maintenance including set-up on manual lathes, vertical mills, heat treating ovens, threading machines, band saws, and hydraulic presses. A hazardous energy program to control the release of stored energy for machinery with multiple sources had not been established and implemented.
