29 CFR 1917.50(c)(5)(iii): Every spreader not a part of the ship's gear and used for hoisting intermodal containers was not inspected and tested to a proof load equal to 25 percent in excess of its rated capacity before initial use.    a) Port of Pago Pago, American Samoa main dock.  On or about 26 APR 2012 and at times prior thereto, a load engaging spreader used for lifting 20 and 40 foot containers was not tested and marked with a safe working load (SWL) label exposed workers to crushing hazards.

On 26 APR 2012 and at times prior thereto, employees were allowed to a drive forklift and a Highlifter F-1, without certification from any formal training that include name of the trainer, operator(s), date of training and evaluation to ensure operating the equipment safely.
