29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a)(LOCATION)(TYPES OF EQUIPMENT USED)(TYPE OF PLATFORM(S))(DESCRIBE HAZARD(S) WHERE NECESSARY) Instance a: On or about May 18, 2012, roof of a building, where the employer did not ensure employees accessing the roof were provided with equipment designed for safe access to work levels. Employees were using the boom extension of a rough terrain forklift truck and the forks/platform to access the roof. The work platform was not specifically designed by the manufacturer for the purpose in which it was used. This practice created conditions which exposed the employees to fall hazards of approximately 9-feet to the ground below. Instance b: On or about May 18, 2012, on the platform, where the employer did not ensure employees accessing the roof were provided with equipment designed for safe access to work levels. Employees were accessing the roof using the boom extension of a rough terrain forklift truck and the forks/platform. The entire platform was not attached to the fork, while the platform was occupied. This practice created condition which exposed the employees to fall hazards of approximately 9-feet to the ground below.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: On or about May 18, 2012, on the work platform, where the employer did not ensure employees were accessing the roof using a mobile scaffold with all casters and wheels locked, to prevent movement of the scaffold. This practice created conditions which exposed the employees to stumbling and falling from the scaffold during mounting and dismounting.
