29 CFR 1926.403(j)(2)(i): Electrical installations having exposed live parts operating at over 600 volts, nominal, were accessible to unqualified persons:    (a) Jobsite, Basement:  On or about May 22, 2012, the wire door to the fenced in electrical room was not locked nor under the observation of a qualified person.  The exposed live energized parts in this room were 2,500 phase to ground and 4,200 phase to phase.  An employee, who was not a qualified person, walked into this electrical room and came in contact with an energized electrical part and received a fatal electrical shock.
