29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a.) EMS Ice, Inc., Warehouse, Panel A, 480v electrical panel. On or about May 23, 2012, employees were inadvertently exposed to live parts due to openings created by missing circuit breakers or blank covers to prevent contact with the energized bus bar in the rear of the panel.

On or about May 23, 2012, the overhead platform used for storage of miscellaneous items, and inventory did not have the load capacity posted.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a.) EMS Ice, Inc., Shop/Warehouse Area, mini-kitchen area. On or about May 23, 2012, a temporary flexible cord with a multi-outlet female plug was being used in lieu of permanent wiring in that it was providing electrical service for a microwave and refrigerator.
