OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees were exposed to a struck by hazard while starting up a steam turbine that was not protected against over presurization by an opeartional relief valve:      (a) On or about June 1, 2012, in the Sulfuric Acid Plant #3, employees attempting the restart up of a tripped steam turbine were struck by flying metal debris when the turbine housing ruptured due to an apparent overpressurization from 550 psi steam. The overpressurization of the 550 steam line was the result of a pressure relief valve not functioning or lifting to relieve steam pressure.
