29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On site of the new Big John Monahan Bridge being built on SW Warfiled Boulevard in Indiantown, Florida: An employee performing work on a walking/working surface approximately 14 ft. 9 inches above a lower level and exposed to potential fall hazards was not provided with a training program that would enable him to recognize the hazards of falling. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On site of the new Big John Monahan Bridge under construction on SW Warfield Boulevard in Indiantown, Florida: An employee performing work plumbing up a concrete pile to be driven through a steel template he was standing upon approximately 14 ft. 9 inches above a lower level with unprotected sides and edges was not protected from falling to that lower level, on or about June 7, 2012. Cone & Graham, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 314964008, citation number 1, item number 3, and was affirmed as a final order on October 6, 2010, with respect to a workplace located at 3602 N U.S. Highway 301 in Tampa, Florida.
