29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures: On or about June 13, 2012, a 120 volts breaker box in the middle of the building had an unprotected opening through which the power cords to the temporary lights ran. This condition exposed the employees to electrical shock hazards.
