29 CFR 1910.216(a)(4):     New mill installations were not installed so that the top of the operating rolls was not less than 50 inches above the level where the operator was standing, irrespective of the size of the mill:    Lip Seal Production Area, 10 Forbes Rd, Northborough, MA    On 6/18/12 and at times prior, employees operated a Farrell-Birmingham rubber mill with the top of the mill rollers 46 inches above the work surface, exposing employees to the crushing hazards of the in-running nip point between the two rollers.

29 CFR 1910.216(b)(1):     Safety trip control. A safety trip control was not provided in front and in back of each mill.  The safety trip control was not one of the three types specified in 29 CFR 1910.216(b)(1)(i), (ii), and (iii).    Lip Seal Production Area, 10 Forbes Rd, Northborough, MA    On 6/18/12 and at times prior, employees operated a Farrell-Birmingham rubber mill that did not have the required safety trip controls, exposing them to crushing hazards of the in-running nip point between the two rollers.
