29 CFR 1926.416(a)(3):     Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit    On or about July 3, 2012:    (a) Employees were engaged in the installation of electrical equipment to provide power to space heaters used to regulate condensation within the switchgear cabinet located within a substation, were exposed to electrical shock and arc flash hazards. As employer failed to establish through inquiry, direct observation or testing that electrical equipment was energized.

29 CFR 1926.957(a)(2)(i):     When work was to be done in an energized substation, a determination was not made as to what facilities were energized at the time of the work.    On or about July 3, 2012:    (a) Employees were engaged in the installation of electrical equipment to provide power to space heaters used to regulate condensation within the switchgear cabinet located within a substation.  The employees were exposed to electrical shock and arc flash hazards as the employer failed to ensure that electrical components were properly identified as energized before work on electrical components was performed.
