29 CFR 1926.1053(b)(1):     Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support and a grasping device such as a grabrail was not providel.    a) At 1000 Jerry Saint PE Hwy in Pascagoula, MS -- On or about June 26, 2012, the extension ladder, used to access a steel beam at about 27 feet above the ground, extended about 18 inches above the landing and there was no grabrail provided.
