29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sructure, personnel hoist, or similar surface was not used.    (a)  On June 29, 2012, the employer did not ensure that a safe means of access from the roof top area to the Self-Propelled Elevated Work Platform, in that employees were allowed to jump from the roof onto the platform approx. 4 feet below. Employees were exposed to serious injuries from a potential fall or tumble as they attempted access to the Elevated Work Platform.       Abatement certification for this violation is not required.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  (a)  On June 29, 2012, the employer did not ensure that a complete Guardrail System was installed along all open ends of the Self-Propelled Elevated Work Platform, in that Midrail Component of the system had not been attached to complete the requirements of a Guardrail System. Employees were exposed to serious injuries from a potential fall to lower levels.        Abatement certification for this violation is not required.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:  (a)  On June 29, 2012, Employees engaged in work activities involving the use of Self Propelled Elevated Work Platforms showed indications that they had not maintained an understanding and knowledge relating to Fall Protection and Safe access requirements. Employees were exposed to potential serious injuries as a result of needed refresher training.        Abatement certification is required for this item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)  On June 29, 2012, two employees engaged in welding work on a commercial construction project, were not protected from falls to lower levels. The employees were exposed to serious injuries from fall hazards of approx. 17 feet one half inch to the ground level.      Abatement certification for this violation is not required.
