29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees.    WORKSITE: The employee wearing sneakers while performing roofing activities which included stepping from the access ladder on the porch roof onto the debris pile surrounding the bottom of the ladder.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     WORKSITE: The employees performing roofing activities on a 10 in 12 pitch roof, were not using an appropriate fall protection system, exposing them to a 20 foot fall to the ground.

WORKSITE: The 20 foot aluminum extension ladder, used by employees to access the porch roof, was setup at an angle where the horizontal distance of the top support to the foot of the base of the ladder exceeded one-quarter (1/4) the working length of the ladder.

WORKSITE: The 20 foot aluminum extension ladder, used by employees to access the main roof was missing a slip resistant foot, and the ladder was not removed from service.
